Job summary

The Ohio Council of Behavioral Health & Family Services Providers in Columbus seeks a Licensed Adult Behavioral Health Therapist (LISW/LPCC) to join an integrated care team. Qualifications

• Clinically licensed in the state of practice with 3+ years of experience in a behavioral health related field including social work, mental health, and/or addiction.
• Licenses must allow for clinical practice and may include LISW/LPCC; 2+ years of experience in a community mental health center, FQHC, inpatient psychiatric facility or similar setting serving complex behavioral health needs.
• Experience with complex government-sponsored populations preferred (e.g., Medicaid, Medicare).
• Experience with member engagement, transitions of care, interdisciplinary clinical care and/or case management preferred.
• Telehealth experience preferred.
• Excellent clinical judgment and ability to independently manage a caseload.
• Advanced computer skills with Outlook, Word, PowerPoint, Excel.
• Excellent written and oral communication skills, including adapting communication to the member’s needs.
• Experience working in multiple electronic health records.
• Excellent organizational and time management skills to triage and prioritize patient needs.

Responsibilities

• Provide individual, group, post-discharge follow up behavioral health therapy with SMART goals and timely documentation.
• Provide crisis intervention and de-escalation for members in crisis (via phone or in person).
• Serve as the clinical behavioral health SME within ICT with recommendations, referrals, and follow up for behavioral health aspects of care.
• Provide trauma-informed care, harm-reduction, motivational interviewing, and CBT therapies as needed.
• Maintain close communication with PCP and psychiatrist for all therapy patients.
• Complete documentation of patient care services in the EHR within 3 business days of interaction.
• Develop and/or support treatment plans for chronic mental illness (e.g., schizophrenia, bipolar, depression, anxiety).
• Recognize opportunities to promote long-acting injectables to support adherence to treatment plans.
• Schedule appointments and address barriers to attendance (transportation, reminders).
• In collaboration with leadership, develop and maintain group therapy curriculum for the member population.
