Qualifications

• Master's degree in social work (Required).

Responsibilities

• Conduct patient, family, and significant other interviews and chart reviews to assess biopsychosocial functioning.
• Develop and implement intervention plans; provide crisis intervention and various forms of therapy; assist in treatment and discharge planning.
• Maintain current knowledge of hospital and community services to support patient functioning and linkages.
• Thoroughly document assessments and interventions in medical records per department standards and regulatory requirements.
