Job summary

Axess Family Services in Ohio is seeking an Outpatient Therapist to deliver behavioral health treatment under clinical supervision. You will perform assessments, develop treatment plans, provide individual, family, and group counseling, and document Medicaid-billable services in the EHR.

You will collaborate with a licensed supervisor, coordinate care with internal and external providers, and engage in ongoing training to maintain Ohio licensure requirements and comply with HIPAA and Medicaid

Qualifications

• Master’s degree in counseling or social work required.
• Active pursuit of independent licensure in Ohio.
• Unrestricted LPC or LSW license to practice in Ohio or pursuing licensure.

Responsibilities

• Conduct clinical assessments within scope of licensure and under supervision.
• Develop, implement, and review individualized treatment plans with supervisory approval.
• Provide outpatient individual, family, and group counseling consistent with OMHAS-certified service definitions.
• Document services accurately and timely in the electronic health record per Medicaid and agency standards.
• Coordinate care with internal programs and external providers to support continuity of care and referrals.
• Provide crisis intervention, risk screening, and safety planning as directed by supervisor.
• Maintain client confidentiality and HIPAA compliance in handling records.
• Participate in required clinical supervision and quality improvement activities.
