Job summary

Chapters Health System is seeking a Part Time Transportation Coordinator in Florida to support Hope PACE. The role focuses on maintaining van schedules, coordinating driver availability, and ensuring safe, efficient transport for participants.

Responsibilities include dispatching drivers, planning routes, and collaborating with the interdisciplinary team to document progress and changes in medical records. This position requires a Florida license and a clean background check.

Qualifications

• High school diploma or GED.
• At least 1 year experience transporting handicapped individuals and the frail elderly.
• Valid Florida Driver's License with a good record.
• Automobile Insurance.
• Clear Level 2 background check.

Responsibilities

• Maintain van schedule and driver availability.
• Advise IDT and manager of participant-specific transportation issues.
• Develop, update and maintain transportation routes.
• Dispatch drivers; coordinate internal and vendor transportation for all participant appointments.
• Follow established schedule; adjust schedules to meet unanticipated changes; determine safest and most efficient pick-up and delivery routes.
• Participate as a member of interdisciplinary team; contribute to care planning and notes in medical records; report changes to Hope PACE staff.

Key Responsibilities:

• Maintain van schedule and driver availability.
• Advise IDT and manager of participant-specific transportation issues.
• Develop, update and maintain transportation routes.
• Dispatch drivers; coordinate internal and vendor transportation for all participant appointments.
• Follow established scheduled, adjust schedules to meet unanticipated changes, determine the safest and most efficient pick-up and delivery routes.
• Participate as a member of interdisciplinary team. Take part in participant care planning and contribute to notations in medical record. Report important information to proper staff of Hope PACE daily regarding any changes, progress, or unusual events concerning any participant.

Knowledge & Skills:

• Clear communication; via phone, email, and in person.
• Proven attention to detail.
• Demonstrate time management and organizational skills.
• Must be flexible to work in other locations if needed.

Compensation Pay Range:

$18.43 - $27.43
