A Transporter is integral to delivering comprehensive and integrated care. We are seeking a Transporter to join our team that wants to be an integral part of an interdisciplinary care team (IDT) changing lives every day. The role of the Transporter is to provide safe and reliable transportation services for PACE program participants. The Transporter will assist with the overall transportation coordination needs of participants, including scheduling rides and communication as part of the interdisciplinary team (IDT).

The capability to demonstrate the knowledge and skills necessary to participate in providing transportations services to elderly and frail participants according to the assigned responsibilities of the PACE program standards is required. Responsibilities may include assisting participants with simple activities of daily living (ADLs) in preparation for safe transport.

General
  • Knows and adheres to the philosophy and goals of (PACE Program).
  • Maintains a quiet and safe environment for clients, visitors, and staff.
  • Keeps confidentiality of client records, reports, and discussions.
  • Participates in formulation and maintenance of (PACE Program) policies and procedures.
  • Participates in (PACE Program) Committees as requested by the Clinical Services Director.
  • Actively participates in Interdisciplinary Team (IDT) meetings, as appropriate.
  • Attends and participates in scheduled staff meetings and client care meetings as requested.
  • Advises the Director of Clinical Services in ways and means to establish better accountability of (PACE Program) services to clients and referral sources.
  • Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
  • Utilizes supplies and equipment economically.
  • Informs the Clinical Services Director of “unusual incidents.”
  • Identifies service delivery problems and uses good judgment in their solution.
  • Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Clinical Services Director as appropriate; responds in a positive manner to constructive criticism; serves as a role model for students and staff members.
  • May serve as a role model for new employees.
  • Performs other duties as required.
Participant Care
  • Effectively plan and organize assignments in the most efficient manner.
  • Assists in planning daily transportation routes with the PACE program transportation, within the prescribed geographic radius. Participates in planning and coordinating most efficient use of PACE program vehicles.
  • Transports participants in a safe manner to and from the Center on each day they are scheduled to attend, in compliance with Kentucky Department of Motor Vehicle licensing regulations and local statutes.
  • Provides physical assistance to each participant when needed as he/she boards and disembarks from the program’s vehicles.
  • Provide minor support to participants with ADLs to ensure safe transport.
  • Ensures that emergency medical information is available for those participants being transported.
  • Maintains the first aid kit in the vehicle, replacing supplies as they are used of become outdated.
  • Ensures that all participants wear safety (seat) belts and wheelchairs are locked into position.
  • Maintains vehicle in neat and clean condition anticipating needed mechanical service so appointments can be arranged that are convenient to program operations. Keep vehicle maintenance records current; coordinate and schedule routine and episodic vehicle maintenance.
  • Immediately reports to Clinical Services Director any vehicle operating problems, malfunctions, or incidents out of the ordinary routine.
  • Maintains accurate daily records of number of trips, individuals transported, miles and gallons of gas. Compiles all necessary information to meet reporting requirements.
  • Transports participants to and from physician or other medical appointments, as requested by the IDT via the care plan.
  • Assists with program activities by transporting participants to an event, as coordinated by the Activities Coordinator.
  • Supports Quality Management Program through participation in quality studies, provision of recommendations for improvement, and active participation in performance improvement actions.
Licensure / Certification Requirements
  • Current valid Kentucky driver’s license with good driving record.
  • Current CPR Certification.

  • The ability to form positive interpersonal relationships with a wide range of staff and clients.
  • The ability and manual dexterity to perform wheelchair and ambulatory transfers between home and van, and Center and van.
Desired or Preferred Knowledge, Skills and/or Abilities
  • Some experience working with the elderly is desirable.
  • Ability to establish and maintain cooperative working relationships.
  • Ability to remain flexible, adaptable, and able to function in stressful situations.
  • Acceptable physical and mental health to carry out the responsibilities of the position.
