Job summary

Chapters Health System is seeking a dedicated driver to provide safe, reliable transportation for PACE participants in Cape Coral, FL. You will operate a van, assist frail passengers, and maintain a clean vehicle while coordinating with caregivers and medical staff.

The role requires a valid Florida driver’s license, auto insurance, and First Aid/CPR/AED certification; a background check will be conducted. Work days are Monday–Friday with day shifts.

Qualifications

• High School Diploma required.
• Valid Florida Driver’s License and auto insurance.
• First Aid/CPR/AED certification with hands-on skills validation.
• Alzheimer’s Disease and Related Dementias Training per Florida Dept. of Elder Affairs.

Responsibilities

• Transport handicapped passengers safely and on time to venues and medical appointments.
• Maintain vehicle check and report maintenance needs.
• Assist passengers on/off the van and manage mobility aids.
• Follow scheduled routes and adapt for changes while ensuring safety.
• Communicate with caregivers, staff, and Hope PACE daily about changes.

Schedule-Monday-Friday-days

Provide safe, reliable, and courteous transportation to the PACE center and to medical appointments. Must have a clean driving record and be able to pass a level 2 background check. Must have great people skills.

Educational/Training Requirements:

• High School Diploma

Certificates, Licenses, and Registration:

• Must possess a valid Florida Driver’s License, current auto insurance, and reliable transportation.
• First Aid, CPR, and AED certification is required. Course certification must include hands‑on skills validation to ensure competency. Courses from both the American Red Cross (Adult First Aid/CPR/AED) or the American Heart Association (Heartsaver® First Aid CPR AED), meet this requirement
• Certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Experience Requirements:

• At least one year experience in transporting handicapped individuals and working with the frail elderly.

Position Specific Responsibilities: (not all inclusive)

• Under the supervision of the Manager, Transportation, is responsible for the direct transportation of physically limited passengers to and from their destinations. Position required constant interaction with caregivers and community agencies while conducting business with Hope PACE.
• Transport physically limited passengers to varied destinations both timely, safely and door to door.
• Ensure that the van is in proper working order. Report and document maintenance performed.
• Assist passengers on and off the van in a safe and courteous manner. Deal with medical equipment and other emergencies that may arise.
• Follow established scheduled, adjust schedules to meet unanticipated changes , determine the safest and most efficient pick up and delivery routes.
• Participate as a member of interdisciplinary team. Take part in participant care planning and contribute to notations in medical record. Report important information to proper staff of Hope PACE daily regarding any changes, progress, or unusual events concerning any participant.
• Participate in the maintenance of a safe, clean and healthful day center environment.
• May assist with patient care in the day health center when not transporting participants.
• Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.
• Consistently represents Hope with integrity and professionalism.
• Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service.
• When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor.
• Assist with special projects, as requested.
• All other job duties as may be assigned.

Compensation Pay Range:

$16.96 - $25.24
