Job summary

Chapters Health System in Florida is seeking a part-time Transportation Coordinator to maintain van schedules, coordinate drivers, and manage participant transportation for Hope PACE and related appointments.

The role supports the interdisciplinary care team, ensuring timely pickups and safe transport. A valid Florida driver's license, automobile insurance, and a clear background check are required. Workdays are Monday–Friday with daytime hours.

Responsibilities

• Maintain van schedules and driver availability.
• Advise IDT and manager of participant-specific transportation issues.
• Develop, update and maintain transportation routes.
• Dispatch drivers; coordinate internal and vendor transportation for all participant appointments.
• Follow established schedules; adjust schedules to meet unanticipated changes; determine safest and most efficient pick-up and delivery routes.
• Participate as a member of the interdisciplinary team; contribute to participant care planning and document updates in medical records.

Key Responsibilities

• Maintain van schedule and driver availability.
• Advise IDT and manager of participant-specific transportation issues.
• Develop, update and maintain transportation routes.
• Dispatch drivers; coordinate internal and vendor transportation for all participant appointments.
• Follow established scheduled, adjust schedules to meet unanticipated changes, determine the safest and most efficient pick-up and delivery routes.
• Participate as a member of interdisciplinary team. Take part in participant care planning and contribute to notations in medical record. Report important information to proper staff of Hope PACE daily regarding any changes, progress, or unusual events concerning any participant.

Required qualifications

• High school diploma or general education degree (GED).
• At least one year experience in transporting handicapped individuals and working with the frail elderly.
• Valid Florida Drivers’ License with good record
• Automobile Insurance
• Clear level 2 background check

Compensation Pay Range: $18.43 - $27.43
