Job summary

Chapters Health System seeks a Transportation specialist to safely transport handicapped and frail elderly passengers to appointments and centers. You will operate a van, assist with boarding, and communicate with caregivers to coordinate timely pickups and drop-offs.

Responsibilities include maintaining vehicle readiness, documenting issues, and following care plans. A valid Florida license, insurance, and first aid/CPR/AED training are required, with a background check and 1+ year of related

• Must possess a valid Florida Driver's License, current auto insurance, and reliable transportation.
• First Aid, CPR, and AED certification is required.
• Certification of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
• At least one year experience in transporting handicapped individuals and working with the frail elderly.
• Must have a clean driving record and be able to pass a level 2 background check.

• Transport physically limited passengers to varied destinations, timely and safely.
• Ensure the van is in proper working order and report maintenance.
• Assist passengers on and off the van in a safe and courteous manner.
• Follow scheduled routes and adjust for unanticipated changes as needed.
• Collaborate with interdisciplinary team and document pertinent notes.

Courses from both the American Red Cross (Adult First Aid/CPR/AED) or the American Heart Association (Heartsaver First Aid CPR AED), meet this requirementCertification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.Experience Requirements:At least one year experience in transporting handicapped individuals and working with the frail elderly.Position Specific Responsibilities: (not all inclusive)Under the supervision of the Manager, Transportation, is responsible for the direct transportation of physically limited passengers to and from their destinations. Position required constant interaction with caregivers and community agencies while conducting business with Hope PACE.Transport physically limited passengers to varied destinations both timely, safely and door to door.Ensure that the van is in proper working order. Report and document maintenance performed.Assist passengers on and off the van in a safe and courteous manner. Deal with medical equipment and other emergencies that may arise.Follow established scheduled, adjust schedules to meet unanticipated changes , determine the safest and most efficient pick up and delivery routes.Participate as a member of interdisciplinary team. Take part in participant care planning and contribute to notations in medical record. Report important information to proper staff of Hope PACE daily regarding any changes, progress, or unusual events concerning any participant.Participate in the maintenance of a safe, clean and healthful day center environment.May assist with patient care in the day health center when not transporting participants.Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.Consistently represents Hope with integrity and professionalism.Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job.
