Job summary

Reach for Resources in Minnesota seeks a Transportation Coordinator to manage Trail by Reach and Reach in Motion programs, enabling adults with developmental disabilities to access community services. You will coordinate rides, monitor Lyft accounts, budget allocations, and billing, while ensuring safety and reliable service.

You will work remotely with occasional in-person meetings, liaising with riders, caregivers, and providers to optimize scheduling and outcomes, and supporting participants

Qualifications

• High school diploma or equivalent; relevant post-secondary education preferred.
• Strong organizational and time-management skills.
• Effective communication with diverse stakeholders, including individuals with disabilities.
• Proficiency with Microsoft Office and mobile applications; learn Lyft and scheduling systems.

Responsibilities

• Oversee coordination and operations of the Trail by Reach and Reach in Motion transportation programs.
• Communicate with riders, caregivers, and drivers to confirm logistics and maintain a reliable transportation experience.
• Submit rider lists to third-party providers and manage changes or cancellations.
• Ensure eligibility and conduct assessments for Trail by Reach riders.
• Assist participants in setting up Lyft accounts and using the Lyft app.
• Set up Lyft Pass allocations tied to county waiver budgets; ensure compliance.
• Respond to transport issues and document service metrics and outcomes.
• Maintain records related to rider information, funding sources, and logistics.
• Complete monthly billing in Procentive.
• Collaborate with other departments to align transportation with schedules and needs.
• Provide training materials related to transportation services as needed.
