Reach for Resources, Inc. seeks a Transportation Coordinator to oversee two programs: Trail by Reach and Reach in Motion, managing ride logistics and ensuring safe, efficient service.

The role involves coordinating with riders, caregivers, and drivers, helping participants set up Lyft accounts, and monitoring Lyft Pass allocations. This position operates largely remotely with occasional on-site meetings as needed.

Qualifications

• Proficiency with Microsoft Office Suite and mobile apps.
• Ability to learn Lyft, county waivers, and scheduling systems.
• Valid driver's license and reliable transportation.
• Must pass an MNDHS background check.

Responsibilities

• Oversee the coordination and operations of the Trail by Reach and Reach in Motion transportation programs.
• Communicate with riders, caregivers, and drivers to confirm logistics and maintain a reliable transportation experience.
• Submit rider lists to third-party transportation providers and manage changes or cancellations.
• Ensure all Trail by Reach riders meet eligibility requirements and conduct assessments as needed.
• Provide support and guidance to participants setting up Lyft accounts and using the Lyft app.
• Set up and monitor Lyft Pass allocations tied to county waiver budgets.
• Respond to and resolve transportation-related issues including ride access and Lyft funds visibility.
• Track and document participant usage, program metrics, and service delivery outcomes for reporting.
• Maintain records related to rider information, funding sources, and logistics.
• Complete monthly billing in Procentive.
• Collaborate with other departments to align transportation with schedules and needs.
• Provide training or informational materials related to transportation services.
