Job summary

Mass General Brigham is seeking a Registered Nurse Case Manager for the Surgery service at 55 Fruit Street, onsite in Boston. The role requires collaboration across care teams to coordinate patient care along the continuum, with emphasis on efficient planning, advocacy, and discharge planning.

40 hours/week, day shifts, with weekend coverage as needed. Required: MA RN license, 2+ years Med/Surg, ADN (BSN preferred), Case Manager certification preferred; acute hospital experience preferred.

Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.
• ADN required, but BSN strongly preferred.
• Acute hospital experience preferred.

Responsibilities

• Collaborates to enhance delivery of patient care along the continuum.
• Evaluates, plans, implements, coordinates, monitors, and advocates for care.
• Onboarding and professional development pathways through the Novice to Expert program.
• Supports efforts to ensure reimbursement and cost reduction while maintaining quality.
