Job summary

Mass General Brigham is seeking an RN Case Manager in Transplant to enhance patient care across the continuum. The role focuses on improving patient experiences and ensuring high-quality care within a leading academic hospital.

Responsibilities include collaborating with the healthcare team, optimizing care delivery, and guiding patients through assessment, planning, and coordinate care transitions. Strong Med/Surg background and MA RN licensure are required.

Qualifications

• ADN is required; BSN strongly preferred.
• Minimum of 2 years Med/Surg experience.
• RN Case Manager experience required.
• Massachusetts RN license required; case manager certification preferred.

Responsibilities

• Collaborate with the healthcare team to enhance patient care services.
• Continuously improve the patient experience and ensure institutional standards.
• Evaluate, predict, and facilitate the trajectory of patient care.
• Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
• Participate in an in-depth orientation and onboarding program.
• Engage in the "Novice to Expert" program for professional development.
• Utilize resources from strong leaders and case management specialists.
