Allina Health is a not-for-profit health system serving Minnesota and western Wisconsin. We are seeking a clinical leader to manage care for transplant candidates and organ failure patients across inpatient, clinic, home and community settings.

You will guide policy, protocol and care systems for this patient population and collaborate with a multidisciplinary team. Responsibilities include coordinating assessments, education for patients and families, hospital and clinic care coordination, and

Qualifications

• Bachelor's degree graduate of accredited School of Nursing.
• 2–5 years of clinical management of patients with organ failure/transplant candidates.
• Master's degree preferred.

Responsibilities

• Coordinates assessment, referral and intake of patients entering program.
• Performs initial assessment of patient according to program protocols.
• Collaborates with physician/NP/PA to determine diagnostic testing and follow-up.
• Provides education to patient and family regarding diagnosis and post-hospital self-care.
• Coordinates care of patient in the clinic setting.
• Reviews medical records, orders and schedule prior to visit.
• Assesses patient status during clinic visit.
• Provides after-visit instructions based on plan of care.
• Coordinates care during hospital admission.
• Monitors patient progress.
• Facilitates discharge planning and follow-up.
