Allina Health, a not-for-profit health system serving Minnesota and western Wisconsin, is seeking a Nurse Practitioner for the Advanced Heart Failure team at the Minneapolis Heart Institute in Minneapolis. This role is primarily clinic-based (about 80%) with some weekend rotations and collaborative physician/nurse team care.

The position requires an NP degree with MN RN licensure, NP certification, ACLS/BLS, and DEA registration.

Qualifications

• Master's degree from accredited Nurse Practitioner program.
• Licensed RN in Minnesota upon hire.
• APRN NP certification (ANCC or AANP) upon hire.
• ACLS/BLS Tier 2 certification within 180 days.
• DEA registration required upon hire.
• Valid driver's license (MN/WI residents must obtain in state within 60 days).

Responsibilities

• Provides clinical care for patients.
• Completes comprehensive patient assessments.
• Determines diagnosis and treatment of illness.
• Orders and interprets medical diagnostics in tandem with attending physician.
• Prescribes medications, treatments, therapy and medical equipment.
• Conducts patient education when working to manage chronic illness or wellness care.
• Refers for specialty care when determined.
• Consults with physician when patient needs outside of Nurse Practitioner scope of practice, diagnosis or treatment plan is unclear.
• Participates in health care team.
• Promotes communication within the team and other interdisciplinary health care staff.
• Acts as resource to members of the team.
• Works with primary care team for program development.
