Qualifications
- Bachelor's degree in nursing is required.
- Licensed Registered Nurse in MN or WI is required.
- Case/Care Management Certification within 2 years of hire (obstetric/cancer exceptions apply).
Responsibilities
- Coordinate clinical transition planning for medically complex patients.
- Assess, plan and intervene to support discharge and follow-up.
- Participate in care rounds and ensure timely progression of care.
- Identify barriers to discharge and work with leadership to resolve.
- Collaborate with interdisciplinary teams and community resources.
