We have performed more than 2,200 transplants.Nursing Excellence: We were the first hospital in the region to become a Magnet recognized hospital.**What you will do:*** Coordinate care with post-transplant patients.* Facilitates clinic flow, transplant evaluation, and patient care management.* Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.* Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.* Transplant Coordinator will manage a case load of patients, remain patient’s point of contact through the transplant process, recovery and at home.* Registered Nurse, BSN should have 2+ years of acute care experience, good medical - renal knowledge, and understands chronic disease. BLS required. Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.**Outpatient clinic is open Monday – Friday between 8am and 4:30pm.
