Job summary

Baylor St. Luke’s Medical Center, part of CommonSpirit Health, seeks an RN Transplant Coordinator to orchestrate the transplant process from referral through recovery.

You will manage referrals, evaluate candidates, support in-hospital care, education, and stakeholder communication, while maintaining critical data and promoting patient advocacy. The role requires strong clinical expertise, organization, and communication, with a focus on program excellence and patient outcomes.

Qualifications

• BSN required with three years’ experience in acute care/ICU, dialysis, or transplant.
• RN licensure in TX required at hire.
• Basic Life Support (CPR) certification due within 14 days of hire.

Responsibilities

• Review medical information, schedule testing, and compile data for patient case presentations.
• Notify consultants and coordinate rounds, communicating with patients and all disciplines involved.
• Maintain effective communication with team and families throughout care.
• Lead pre- and post-op education with nursing staff to ensure comprehensive teaching.
• Support research protocols, publications, and patient education materials.
• Coordinate donor/recipient processes, handle 24-hour donor call and outpatient issues.
