Job summary

Saint Joseph's Hospital and Medical Center in Phoenix, AZ is seeking an RN Transplant Coordinator to guide patients from referral through recovery. You will manage referrals, candidate evaluations, in-hospital care coordination, education, and stakeholder communication, while upholding patient advocacy and program excellence.

This role requires a nursing degree, AZ RN license on hire, and at least three years of related clinical experience; a bachelor’s degree is preferred and critical care

Qualifications

• Graduate of accredited school of nursing.
• Three (3) years experience with patients in a related setting or three (3) years experience in an intensive care, acute care, telemetry or medical/ surgical setting.
• Registered Nurse: AZ, upon hire.

Responsibilities

• Organizes the clinical program
• Develops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient population
• Develops, implements, and maintains ongoing continuous quality improvement activities for the clinical program
• Serves as resource to hospital staff regarding patient care
• Coordinates the patient evaluation process
• Performs initial assessment of patient and family according to protocol for acceptance into program

Education

Accredited nursing program

Bachelor's degree in Nursing (preferred)
