Job summary

Saint Joseph's Hospital and Medical Center (SJHMC) in Phoenix, AZ seeks an RN Transplant Coordinator to guide referrals, candidate evaluations, and in-hospital care coordination for transplant patients. You will facilitate education, payer communications, and critical data management.

The role requires nursing leadership, meticulous coordination, and a patient advocacy focus to drive program excellence and integrated care across stakeholders.

Qualifications

• Graduate of accredited school of nursing.
• Three (3) years experience with patients in a related setting or three (3) years experience in an intensive care, acute care, telemetry or medical/surgical setting.
• Registered Nurse: AZ, upon hire.
• Bachelor's degree or higher preferred.
• Critical care experience preferred.
• Program Management/Development experience preferred.

Responsibilities

• Organizes the clinical transplant program.
• Develops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient population.
• Develops, implements, and maintains ongoing continuous quality improvement activities for the clinical program.
• Serves as resource to hospital staff regarding patient care.
• Coordinates the patient evaluation process.
• Performs initial assessment of patient and family according to protocol for acceptance into program.
