Job summary

Mayo Clinic in Minnesota is seeking a Transplant Coordinator Nurse to manage coordination of transplant patient care across the continuum. You will collaborate with patients, families, and a multidisciplinary team to plan and educate, ensuring needs are met throughout the care journey.

The role requires an RN license, a BSN or ACEN/CCNE-accredited program, and at least 1 year of RN clinical experience (3 years preferred).

Qualifications

• Must have RN license by applicable state requirements.
• Graduate of a BSN or ACEN/CCNE-accredited nursing program.
• Minimum 1 year of clinical RN experience required; 3 years preferred.

Responsibilities

• Coordinate all aspects of care for transplant patients.
• Collaborate with patient, caregiver, and multidisciplinary team to plan and deliver care across the continuum.
• Assess patient data and interpret information relative to age-specific needs.
• Provide patient and family education consistently.
• Demonstrate leadership through community involvement, committees, and quality improvement.
