Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse to oversee coordination of care for transplant patients. You will collaborate with patients, families and a multidisciplinary team to plan and implement care, ensuring education and support across the care continuum.

Applicants must hold a BSN from ACEN/CCNE accredited program with RN licensure, and have at least 1 year of clinical RN experience (3 years preferred). The role is full-time with on-site duties and competitive benefits.

Qualifications

• Graduate of a baccalaureate ACEN or CCNE accredited nursing program.
• Minimum 1 year of clinical RN experience required; 3 years preferred.

Responsibilities

• Coordinate care for transplant patient population in collaboration with patient, caregiver and multidisciplinary team.
• Assess data and plan to meet age-specific needs; provide education to patient and family.
• Lead quality improvement and research activities according to ANA/UNOS/FACT guidelines.
