Mayo Clinic is seeking a Transplant Coordinator Nurse in Minnesota to coordinate all aspects of care for the transplant patient population. You will assess and plan with patients, caregivers and a multidisciplinary team to meet needs across the care continuum and provide consistent education to patients and families.

The role requires RN licensure, 1 year of clinical RN experience (3 years preferred), and certification/continuing education obligations including BLS and transplant-related

Qualificações

• RN license by applicable state requirements.
• Minimum 1 year of clinical RN experience; 3 years preferred.
• Graduate of ACEN/CCNE-accredited nursing program.

Responsabilidades

• Coordinate all aspects of care for transplant patient population in collaboration with patient, caregiver and multidisciplinary team.
• Assess data and interpret information for age-specific needs.
• Provide patient and family education consistently.
• Lead through participation in quality improvement, research and committee work.
