Job summary

Responsibilities

• Collaborates with multidisciplinary team to develop and implement individualized care plans.
• Maintains thorough and confidential patient charts and records.
• Facilitates communication regarding UNOS listing, organ placement, and recipient admission.
• Coordinates transplant process including pre- and post-transplant care.
• Educates patients and families on transplantation and care plans.
