Job summary

Devoted Health, Inc. is seeking a Nurse Practitioner/Physician Assistant to lead the Transitions of Care (TOC) program as APP Manager. This remote, full-time leadership role combines clinical care with operational oversight of the APP team to improve post-discharge outcomes.

The ideal candidate has 5+ years in clinical practice, 2+ years in management, and state licensure for multiple states. You will mentor staff, develop KPIs, and partner with the Medical Director to advance TOC processes.

Qualifications

• Active RN and APRN license with national APRN certification or PA license in home state, and ability to obtain multi-state licensure.
• 2+ years of management or team leadership experience.
• 5+ years in clinical practice as NP/PA with post-discharge patient care experience.

Responsibilities

• Provide TOC patient care 40% of the time, focusing on 30-day post-discharge gaps and readmission reduction.
• Lead APPs in the TOC program, manage performance and KPIs, and participate in TOC leadership team.
• Oversee recruitment, onboarding, and professional development of TOC APPs, and collaborate with Medical Director to optimize workflows.
