Job summary

Elara Caring in Oklahoma City is seeking an Admission Nurse to support our home health patients with compassionate, compliant care. You will assess hospice patients, complete initial evaluations, and deliver care per the individualized Plan of Care to ensure quality and continuity.

Qualifications include an active RN license in Oklahoma, 1+ year of clinical experience, ability to lift up to 50 pounds, and a valid driver's license or reliable transportation to visit patients.

Qualifications

• Current, unrestricted state RN licensee.
• Graduate of an accredited nursing program (Certificate, Diploma, AD, or BSN).
• 1+ year of clinical care experience as a nurse.
• Ability to lift/push/pull up to 50 pounds and assist with patient transfers.
• Valid driver's license or reliable transportation to patient homes.
• Associates or Bachelors degree in Nursing preferred.
• Home Health or Hospice experience preferred.
• HomecareHomeBase experience preferred.

Responsibilities

• Assess hospice patients to identify physical, psychosocial, environmental, and nursing needs.
• Complete initial evaluations and reassess patient needs and condition during visits.
• Provide care according to the Plan of Care to support quality and continuity.
• Monitor changes in patient condition and adjust the Plan of Care as needed.
• Communicate significant changes to physicians and care team.
• Maintain accurate clinical documentation and protect patient privacy per HIPAA.

• Assess hospice patients to identify physical, psychosocial, environmental, and nursing needs.
• Complete initial evaluations and reassess patient needs and condition during each visit.
• Provide care according to the individualized Plan of Care to support quality, continuity of care, and positive patient outcomes.
• Monitor for changes in patient condition and adjust the Plan of Care or take appropriate action as needed.
• Communicate significant changes, concerns, or safety issues to the physician, Patient Care Manager, and other members of the care team.
• Maintain accurate clinical documentation while protecting patient privacy and confidentiality in accordance with HIPAA requirements.

What is Required?

• Current, unrestricted state RN licensee
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year experience in a clinical care setting as a nurse
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Associates Degree or Bachelors Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with HomecareHomeBase is preferred
