Responsibilities

• Conduct intake interviews and needs assessments for clients.
• Create individualized treatment plans and document services.
• Perform crisis intervention as needed and coordinate referrals.
• Collaborate with internal departments to ensure quality care.
• Assist with housing placement documentation and benefit applications.
• Enter client data into agency databases and prepare discharge reports.
