Job summary

BHcare in Connecticut seeks a Transitional Counselor to support adults with mental health challenges in the Howe Residential Program, providing coaching and skill-building to promote recovery and independent living.

This full-time onsite role runs Monday–Friday, 3pm–11pm, with duties including coordinating care, facilitating groups, advocating for residents, maintaining records, and connecting clients with behavioral health and community resources.

Qualifications

  • Two years of college education in Human Services or related field OR a High School Diploma/GED with 2-4 years of direct care experience.
  • Experience supporting individuals with mental health and co-occurring challenges preferred.
  • Strong communication, organization, documentation, and problem-solving skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Valid Connecticut driver’s license with acceptable driving record and ability to transport clients as needed.
  • CPR, AED, and First Aid certification preferred.

Responsibilities

  • Provide coaching and skill-building in personal hygiene, meal planning, cooking, housekeeping, budgeting, and community living skills.
  • Coordinate care by connecting residents with behavioral health, vocational, educational, healthcare, and community resources.
  • Facilitate rehabilitation and recovery-focused groups and activities.
  • Advocate for residents while helping them establish natural supports and community connections.
  • Supervise self-medication protocols and support residents' health and safety needs.
  • Provide crisis intervention and collaborate with emergency services and treatment providers when necessary.
  • Maintain accurate documentation, progress notes, communication logs, and electronic health records.
  • Transporting clients and supporting community-based activities as needed.

We are seeking a compassionate and dedicated Transitional Counselor to join our Howe Residential Program.The Howe Residential Supervised Housing Program provides quality, recovery-focused services that promote residents' well-being, community integration, and independent living skills.
