Job summary

BHcare is seeking a compassionate Transitional Counselor for the Howe Residential Program in Connecticut. This full-time role supports adults with mental health challenges toward recovery and greater independence, with a 40-hour work week and a scheduled 3pm–11pm shift.

This onsite essential position requires CT residency and reports during emergencies. Responsibilities include coaching in daily living skills, coordinating care with health and community resources, facilitating groups,

Qualifications

• Two years of college education in Human Services, Behavioral Science, or a related field preferred.
• High School Diploma/GED with 2–4 years direct care experience also acceptable.
• Experience supporting individuals with mental health and co-occurring challenges preferred.
• Strong communication, organization, documentation, and problem-solving skills.

Responsibilities

• Provide coaching and skill-building in personal hygiene, meal planning, cooking, housekeeping, budgeting, and community living skills.
• Coordinate care and connect residents with behavioral health, vocational, educational, healthcare, and community resources.
• Facilitate rehabilitation and recovery-focused groups and activities.
• Advocate for residents while helping them establish natural supports and community connections.
• Supervise self-medication protocols and support residents' health and safety needs.
• Provide crisis intervention and collaborate with emergency services and treatment providers when necessary.
• Maintain accurate documentation, progress notes, communication logs, and electronic health records.
• Transport clients and support community-based activities as needed.
