Job summary

Universal Healthcare IPA, Inc. is seeking a Case Management Assistant for Transitional Care Services to support case managers and coordinate post-discharge care. The role focuses on helping members navigate transitions from ER visits, acute care, and other care settings within the ECM program.

The position emphasizes strong interpersonal skills, documentation in CM systems, and collaboration with ICT teams to meet member needs and goals.

Qualifications

• Education: High School diploma or GED required.
• Minimum of 3 years of experience in a health care or community health setting.
• Advanced knowledge of prior authorization or case management regulations governing Medi-Cal, Commercial, Medicare, CCS, and other programs.
• Experience in a managed health care environment preferred (IPA, HMO, or Health Plan).
• Medical Assistant or Community Health Worker certification preferred.

Responsibilities

• Works collaboratively with Case Managers to manage members needing Transitional Care Services.
• Coordinate post-discharge services: appointments, referrals, transportation, and follow-up needs.
• Initiate care transition coordination with referrals and internal partners for seamless transitions home.
• Manage low acuity member cases within the ECM Program.
• Contact members at regular intervals per acuity level and care plan needs.
• Document care management activities in CM System per protocol.
• Assist with care coordination, follow-up, and communications with agencies and reports.
• Report variances to nursing or social services staff assigned to the member.
• Assist with scheduling, transportation, referrals, and other coordination services.
• Gather information from outside sources such as PCPs and other providers.
