Job summary

Universal Healthcare MSO in Bakersfield, CA is hiring a Case Management Assistant – Transitional Care Services (TCS) to support the ECM program. You will coordinate services, assist nurse case managers and social services, and help manage members through transitions from ER visits, acute care, and post-acute care settings.

The role requires strong interpersonal and organizational skills, attention to detail, and the ability to work collaboratively with an interdisciplinary team.

Qualifications

• Education: High School diploma or GED required.
• Experience: Minimum of 3 years in health care or community health setting.
• Knowledge: Prior authorization or case management regulations governing Medi-Cal, Commercial, Medicare, CCS and other programs.

Responsibilities

• Collaborate with Case Managers to manage Transitional Care Services.
• Coordinate post-discharge services and appointments; arrange transportation and follow-up.
• Initiate care transition coordination with referral sources and partners.
• Manage low acuity cases within ECM program.
• Contact members regularly per care plan and document activities in CM System.
• Assist with coordination of referrals, reports, and ICT meetings.
