Universal Healthcare MSO, LLC is seeking a Case Management Assistant for the Transitional Care Services (TCS) program in Bakersfield, CA. This full-time, onsite role supports the Case Management Department and ECM initiatives through coordinating care transitions from ER and hospital settings.

The role emphasizes strong communication, patient coordination, and data collection to ensure smooth post-discharge services and adherence to program protocols.

Qualifications

• High School diploma or GED required.
• Minimum of 3 years of experience in health care or community health.
• Knowledge of Medi-Cal, Commercial, Medicare and related programs.
• Experience in managed health care environment preferred (IPA, HMO, or Health Plan).
• Medical Assistant or Community Health Worker certification preferred.

Responsibilities

• Assist Clinical or Social Services Case Managers to manage Transitional Care Services.
• Coordinate post-discharge services, schedule appointments, and arrange transportation.
• Initiate care transition coordination with referral sources and internal partners.
• Document care management activities in CM System per protocol.
• Assist with data gathering for ICT meetings and reports.
• Attend departmental and staff meetings and help train new staff.
