Job summary

Qualifications

• Bachelor's Degree in Nursing; required.
• RN license active in Alaska with BLS card.
• Three years RN experience; case management preferred.
• Experience with transition and discharge planning.

Responsibilities

• Identifies care management patients and contacts within 24 hours of referral.
• Creates and coordinates discharge plans for high‑risk patients.
• Reviews progress against discharge goals and coordinates with teams.
• Prevents readmissions through multidisciplinary collaboration.
• Coordinates with pre‑authorization and financial counselors as needed.
