Qualifications

• A Bachelor’s Degree in Nursing. Professional work‑related experience, education, or training may be substituted on a year‑for‑year basis for education.
• Non‑supervisory – Three (3) years professional work experience as a Registered Nurse.
• Current registered nurse license in the State of Alaska.
• Current Basic Life Support (BLS) card.
• Experience in the Alaska Tribal Health System is preferred.
• Nationally recognized case management certification is preferred.

Responsibilities

• Identifies care management patients within 24 hours of inpatient or ED referral.
• Creates and coordinates a focused transition and discharge plan of care for chronic, high‑risk patients.
• Conducts concurrent medical record review to measure progress against discharge goals.
• Prepares and presents cases for discussion at scheduled meetings.
• Expedites sequencing and scheduling of interventions and communicates patient needs to the site coordinator.
• Reviews transition/discharge plan to ensure patient and family understand medications and needs.
• Coordinates with pre‑authorization and financial counselors for acute and post‑acute care as needed.
• Identifies and communicates problems affecting patient flow, satisfaction, safety, or length of stay to leadership.
• Works with multidisciplinary team to prevent readmissions and facilitate care conferences.
