Job summary

Southcentral Foundation (SCF) is hiring an RN Case Manager - Inpatient Pediatrics Transitions Coordinator at AK Native Medical Center in Anchorage, AK. This full-time, on-site role focuses on ensuring timely, high-quality care for Alaska Native customer-owners through effective transition planning and care coordination.

The position requires an Alaska RN license, BLS certification, and experience or completion of SCF's RN Case Manager Training Program; incentives include sign-on and relocation

Qualifications

• Graduate of an accredited school of nursing.
• License in the State of Alaska as a Registered Nurse.
• Basic Life Support (BLS) certification is required. ACLS, NRP, ATLS, ALSO and PALS as required for specialty.
• One (1) year nursing experience or successful completion of the RN Case Manager Training Program at SCF.

Responsibilities

• Deliver appropriate, timely, and beneficial care for customer-owners.
• Coordinate case management within established SCF standards and Advance Access principles.
• Promote quality and cost-effective health care outcomes.
