Job summary

Claratel Behavioral Health seeks a Project HEAL Case Manager to support individuals transitioning from hospital to Transitional Housing. You will coordinate safe, clinically informed transitions and ensure continuity of medical, behavioral health, and substance use treatment services.

You will provide engagement, screening, care coordination, and linkage to ongoing treatment through Claratel Behavioral Health and community resources as needed, with collaboration across partners.

Qualifications

• High school diploma or GED.
• At least one year of experience in behavioral health providing case management/community support services.
• Must possess basic computer and typing skills.
• Care coordination skills to assist individuals with accessing/linkage to resources.
• Must have transportation to and from work.
• Bachelor's degree in related fields is preferred.

Responsibilities

• Coordinate discharge planning with hospital or acute care providers and medical home/home health services.
• Conduct transition planning to support movement from facility-based care into residential settings.
• Facilitate enrollment and linkage to outpatient services, including counseling and substance use treatment.
• Coordinate care with residential providers, medical providers, behavioral health clinicians, and community services.
• Assist participants with appointment scheduling, transportation planning, benefits linkage, and service navigations.
• Monitor engagement in treatment and supportive services into housing.
• Participate in multidisciplinary case coordination with Project HEAL partners.
• Support continuity of care during stabilization after discharge.
• Maintain documentation per policy and regulations.
• Complete data entry into HMIS/ClientTrack.
