Job summary

Qualifications

• Bachelor's degree in related field or equivalent experience.
• At least one year of behavioral health case management experience.
• Proficient in EMR, telehealth, and mobile devices.
• Strong care coordination and referral skills.
• Reliable transportation to work and valid driver's license.

Responsibilities

• Carry a caseload of 25-30 clients and visit weekly for first 3 months, then twice monthly.
• Provide care coordination to ensure housing stability and link to services.
• Deliver ongoing support and skills training aligned with treatment plans.
• Gather information to ensure appropriate level of care.
• Recognize crisis intervention needs and notify supervisor.
• Complete timely documentation for all service contacts.
• Participate in weekly team meetings and meet productivity standards.
• Meet with program manager for professional growth and performance.
• Perform additional duties as assigned.
