Job summary

Claratel Behavioral Health is seeking a Project HEAL Case Manager to coordinate transitions from hospital to residential housing, bridging medical, behavioral health, and housing services.

You will provide engagement, screening, care coordination, and linkage to ongoing treatment through Claratel and community resources, ensuring wrap-around care for clients during stabilization and housing placement.

Qualifications

• High school diploma or GED is required.
• At least one year of experience in behavioral health case management.
• Basic computer and typing skills are required.
• Care coordination skills to link clients with services.
• Must have reliable transportation to work.

Responsibilities

• Coordinate discharge planning with hospital/acute care providers.
• Plan transitions from facility-based care to residential settings.
• Link clients to outpatient services including counseling and psychiatric care.
• Coordinate care with residential and medical providers.
• Assist with scheduling, transportation, and benefits linkage.
• Monitor engagement in treatment and housing stability.
• Participate in multidisciplinary case coordination with partners.
• Support continuity of care during post-discharge stabilization.
• Maintain documentation per policies and HMIS data entry.
• Perform other duties as directed by Manager/Director.
