Job summary

San Gabriel Pomona Regional Center is a private, nonprofit agency contracted with the California Department of Developmental Services to provide services and supports to individuals with developmental disabilities in Los Angeles County.

Reporting to the Manager of Transition Services III, the Service Coordinator provides comprehensive case management to individuals ages 13 to 23 and their families, coordinating medical, behavioral health, educational, vocational, and community resources to

Qualifications

Bachelor's degree in a related field required.
At least one year of experience working with individuals with developmental disabilities or a related field.
Master's degree can substitute for the required experience.

Responsibilities

Coordinate services across medical, behavioral health, psychological, educational, vocational, recreational programs, and adaptive devices.
Explain available resources and services to individuals ages 13–23 and their families.
Conduct in-person meetings with individuals served, families, vendors, and agencies to facilitate service planning and coordination.
Develop and implement individualized Program Plans (IPP) tailored to each individual's needs.
Document service plans, meetings, progress, and interim/annual reports within mandated timelines.
