Job summary

Rite of Passage in Torrance, CA is hiring a Transitional Care Coordinator to administer and develop case management services for youth in our Southern California Treatment Program. You will liaise with residents, families, site departments, and placing agencies to ensure seamless care and successful community transition.

Qualifications

• Associates degree in a related field is required.
• Bachelor's degree preferred for advancement.
• At least 1 year of experience with at-risk youth or related settings.
• Ability to obtain and maintain a valid State Driver's License and clean driving record.

Responsibilities

• Administer, develop, and facilitate case management services for residents.
• Serve as liaison between residents, site departments, families, and placing agencies.
• Ensure successful transition of youth back to the community.
