Job summary

San Gabriel/Pomona Regional Center in California seeks a Service Coordinator to provide comprehensive case management for adults served and their families. You will work with families and community resources to ensure high-quality service and positive outcomes.

Responsibilities include coordinating medical, behavioral health, educational, and vocational services; developing IPPs; documenting notes on time; and presenting cases at committees while meeting strict timelines.

Qualifications

• Bachelor's degree in a related field required.
• At least one year of experience with individuals with developmental disabilities.
• Master's degree can substitute for required experience.
• Bilingual in Spanish is preferred.

Responsibilities

• Coordinate services with Regional Center teams for individuals served, including medical, behavioral health, psychological, educational, vocational, recreational programs, and adaptive devices.
• Explain available resources and services to individuals served and their families.
• Consult with teams regarding the needs of individuals served.
• Conduct in-person meetings with individuals served, families, vendors, and agencies for service planning.
• Develop and implement IPPs tailored to each individual's needs.
• Write inter-disciplinary notes and document service plans and progress on time.
• Document interim and annual reports within mandated timelines.
• Initiate purchase of service (POS) for clients ineligible for other funding.
• Present cases at specialized committees as appropriate.
