Job summary

San Andreas Regional Center seeks a dedicated professional to coordinate services for youth transitioning to adult services under the Lanterman Act. The role includes data gathering, IPP/IFSP development, and collaboration with schools, vendors, and IDT members.

Requirements include a Bachelor’s Degree in Social Work (or related) with possible waivers for experience. The position offers comprehensive benefits and a monthly bilingual stipend in California.

Qualifications

• Education/Experience: A Bachelor’s Degree in Social Work, or a Bachelor’s Degree in Social Work plus four years of relevant experience.
• Special Skills and Knowledge: Knowledge of social work principles and techniques; ability to relate to clients and families; report writing and case management.

Responsibilities

• Gathers data and produces initial and ongoing assessments.
• Presents assessment data to IDT for eligibility determination.
• Coordinates the Person-Centered Planning process for individuals and families.
• Develops the IPP or IFSP for Early Start.
• Counsels individuals concerning personal goals and transitions.
• Advocates with individuals/families to assure provision of needed services.
• Monitors compliance with Title 17/Title 22 licensing and DDS requirements.
• Maintains caseload documentation and access to community resources.

Education

Bachelor's Degree in Social Work

Bachelor's Degree in Social Work plus four years of relevant experience may be waived
