Job summary

Bridgeway Community Health in Salem, Oregon seeks a Transition Coordinator I to join the Medically Managed Withdrawal (Detox) team. The role focuses on advocacy during pre-admissions and discharge, serving as a liaison between clients and health care providers to maintain high-quality service.

You will assist with intake, coordinate housing and social services, and ensure smooth transitions by communicating with physicians, case managers, and outside resources.

Qualifications

• Must possess a current CRM certificate or THW credential and be able to pass a background check.
• Strong oral and written communication and organizational skills.
• Ability to work independently and coordinate with multiple providers.

Responsibilities

• Greet clients and handle intake paperwork and reports.
• Coordinate pre-admission transitions and discharge planning.
• Explain care options to patients and families and link to community resources.
• Facilitate housing, transportation and social services support.
• Communicate with physicians, case managers and treatment facilities to ensure smooth transitions.
• Maintain program documentation and data tracking.
