Job summary

Qualifications

• Education: Graduate of an accredited school of nursing or Masters of Social Work.
• Licensure: Current Arkansas or Compact State Registered Nurse license in good standing, if applicable.
• Experience: Minimum of 2 years experience in previous Utilization Review or Case Management preferred.
• Certifications: ACM or CCM Certification preferred.
• Other: Strong communication, organizational, and multitasking abilities. Familiarity with Medicare/Medicaid guidelines and hospital regulatory compliance.
• Preferred Education: Bachelor's Degree

Responsibilities

• SUMMARY: Under the general direction of the Program Director, you will develop an appropriate discharge plan, make recommendations for alternative levels of care, identify cost-effective care options and facilitate appropriate clinical follow-up.
• Collaborate with healthcare team to monitor patient progress and modify discharge plans as needed.
• Provide information to post-hospital vendors to ensure ongoing recovery for the patient.
