Job summary

Evergreen Healthcare Partners, LLC’s StepOne Service is seeking a Service Coordinator to expand visibility near the client hospital and coordinate continuity of care for patients with substance use disorders.

The role works with hospital administrators, nursing leadership, and StepOne Service leadership to plan outreach, build community relationships, and ensure a smooth transition to community-based treatment after acute withdrawal management.

Qualifications

• Experience with substance use disorders and related treatment.
• Familiarity with hospital procedures and social services policies.
• HIPAA guidelines knowledge.
• Minimum 2+ years social service or inpatient experience.
• Minimum 2+ years networking/public relations for community education.
• Self-motivated with the ability to work independently.
• Licensure/certification in social services.
• Bachelor’s degree preferred.
• Located within 60 miles of the client hospital.

Responsibilities

• Functions as a liaison between the contracted hospital and Evergreen Healthcare Partners, LLC.
• Acts as a liaison between the service and the community.
• Professional development – maintaining knowledge of substance use disorders and social service.
