Job summary

Community Bridges, Inc. (CBI) seeks Transition Coordinators to ensure individualized discharge plans and coordinated post-discharge care.

You will schedule follow-up appointments within 7 days and work closely with Utilization Management for authorizations, while delivering counseling and group work to patients and support systems. The role requires a background in behavioral health, healthcare experience, and completion of assessments, with opportunities for growth within CBI's integrated care

Qualifications

• High school diploma or GED required.
• Associate's degree in Behavioral Health preferred.
• Minimum 2 years of full-time health care related work experience.
• Experience with administrative or operational processes in healthcare.
• Experience with specialty populations (SUD, SMI, children, adolescents) preferred.
• Bilingual candidates preferred.
• Valid identification required.
• Behavioral Health Technician (BHT) certification per policy is required.
• Fingerprint clearance card required.

Responsibilities

• Coordinate individualized discharge plans within the CBI system of care.
• Schedule follow-up appointments within 7 days of discharge.
• Perform initial assessments and treatment plans; engage patients and support systems.
• Provide therapeutic services including counseling, group work, discharge planning and authorization coordination.

Skills

Highschool diploma or GED

Associate's degree in Behavioral Heath

2+ years healthcare experience

Administrative/operational healthcare

Experience with SUD/SMI/children/adol.

Bilingual candidates preferred

Valid identification

BHT certification
