Community Bridges, Inc. in Tucson, AZ is seeking a Transition Coordinator-Residential to ensure individualized discharge planning and timely follow-up for clients moving through our behavioral health programs.
The role coordinates with Utilization Management, completes initial assessments and treatment plans, and provides counseling, group work, discharge planning, and authorization coordination. Experience in health care settings and with behavioral health populations is preferred.

Transition Coordinators are assigned at each level of care within the Community Bridges, Inc. (CBI) systems of care network to ensure that patients/patients have an individualized discharge plan. All follow up appointments for ongoing care and services will be scheduled within 7 days of discharge. Transition Coordinators are responsible for coordinating with Utilization Management on all authorizations and continued authorizations. Transition Coordinators will adhere to all policies and procedures. The Transition Coordinator will complete an initial assessment, and treatment plan, as well as engage the individuals and their support system. The Transition Coordinator will provide therapeutic services to individuals and their support systems to include counseling, group work, discharge planning and authorization coordination.
