Job summary

Community Bridges, Inc. (CBI) seeks a Transition Coordinator-Residential to manage patient discharge planning, coordinate post‑care appointments, and work with Utilization Management on authorizations.

Qualifications

  • Highschool diploma or GED required; associate degree preferred.
  • Minimum 2 years of full‑time health care experience exposing you to behavioral and medical contexts.
  • Experience with administrative/operational processes in health care is preferred.
  • Experience with specialty populations (SUD/SMI/children/adolescents) preferred.
  • BIlingual candidates preferred; valid identification required.
  • BHT certification aligned with CBI policies; fingerprint clearance required.

Responsibilities

  • Coordinate discharge planning and ensure follow‑up scheduling within 7 days of discharge.
  • Perform initial assessments and develop treatment plans; engage patients and support networks.
  • Collaborate with Utilization Management on authorizations and continued authorizations.
  • Provide therapeutic services including counseling, group work, and care coordination.

Transition Coordinator-Residential

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare organization offering a full continuum of care, including variety of programs throughout Arizona, Oklahoma and District of Columbia. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Job Summary

Transition Coordinators are assigned at each level of care within the Community Bridges, Inc. (CBI) systems of care network to ensure that patients/patients have an individualized discharge plan. All follow up appointments for ongoing care and services will be scheduled within 7 days of discharge. Transition Coordinators are responsible for coordinating with Utilization Management on all authorizations and continued authorizations. Transition Coordinators will adhere to all policies and procedures. The Transition Coordinator will complete an initial assessment, and treatment plan, as well as engage the individuals and their support system. The Transition Coordinator will provide therapeutic services to individuals and their support systems to include counseling, group work, discharge planning and authorization coordination.

Skills/Requirements
  • Highschool diploma or GED required. Associate’s degree in a field related Behavioral Health preferred.
  • Minimum of 2 years of full‑time health care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical situations required.
  • Experience with administrative or operational processes in a health care setting preferred.
  • Experience working with specialty populations (i.e., SUD, SMI, children, adolescents) preferred.
  • Bilingual candidates preferred.
  • Valid identification required.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures, is required.
  • Fingerprint clearance card required.
