Job summary

Community Bridges, Inc. (CBI) is seeking a Transition Coordinator to support patients across care levels.

You will coordinate discharge plans and ensure follow-up appointments are scheduled within 7 days, working closely with Utilization Management to secure authorizations. Responsibilities include completing initial assessments, crafting treatment plans, engaging patients and their support systems, and providing therapeutic services such as counseling and group work to facilitate seamless

Qualifications

• High school diploma or GED required.
• Associate’s degree in Behavioral Health preferred.
• Experience in healthcare settings and with diverse patient populations is valued.
• Bilingual candidates preferred.

Responsibilities

• Coordinate individualized discharge plans within the CBIs systems of care.
• Schedule follow-up appointments within 7 days of discharge.
• Complete initial assessments and treatment plans; engage patient and support networks.
• Provide therapeutic services including counseling and group work during transitions.
