St. John’s Community Health in South Los Angeles is seeking an Outreach Coordinator to promote HIV prevention services for transgender individuals as part of the Community to Clinic program. The role focuses on outreach and engagement with community and service provider organizations to extend reach in South L.A.

Qualifications

• Minimum of 2 years’ experience working with transgender individuals in a social service setting.
• High School diploma or equivalent; Bachelor's degree preferred.
• Strong facilitation and written/verbal communication skills.
• Ability to work with a multi-disciplinary team.
• Excellent time management and organizational abilities.
• Proficient with MS Word, Excel, Outlook; ability to create program materials.

Responsabilités

• Initiate outreach to target population using telephone, mail, community visits, zoom and other approved avenues.
• Establish relationships with clients and allied organizations.
• Provide education through street outreach, health fairs, school relationships, community events, and other approved activities.
• Attend meetings and maintain communication with program team and supervisor.
• Assist in the production of required weekly, monthly, and/or quarterly data collection reports.
• Maintain outreach activity calendars and logs according to program standards.
• Create, edit, and reproduce outreach materials.
• Prepare meeting packets, notices and minutes.
• Compose correspondence; assist with collecting information for program reports.
