Job summary

Savant Senior Living in Downey, CA is seeking a licensed physical therapist to provide services to residents under a physician’s plan of care. You will assess, plan, and direct therapy, coordinate with families and the care team, and ensure compliance with state practice acts.

The role requires a PT degree, California license, CPR certification, and at least two years of relevant experience; drivers’ license preferred.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Licensed PT by the California Physical Therapy Board.
• Two years of PT experience; home health/community health preferred.
• CPR certification current.
• Must have a valid driver’s license and dependable vehicle.

Responsibilities

• Provide PT services per physician’s plan of care and board guidelines.
• Assess and develop the total plan of care and evaluate functional status.
• Direct physical therapy treatment and supervise related personnel.
• Instruct patients and families in therapy use and appliance care.
• Identify needs for other home health services and refer as needed.
• Submit clinical progress summaries and engagement notes.
